Baden Street Settlement’s Behavioral Health and Community Services provides quality professional services for adults living with mental illness and/or who struggle with substance abuse. The team uses a multi-disciplinary approach to help clients set realistic goals and support them every step of the way. The goal is to help participants achieve independence and community integration.

Behavioral Health Services offers a multitude of services which include Outreach and Care Management for adults as part of Health Home of Upstate New York (HHUNY).

Adult Health Home Care Management Services

Care Managers work closely with individuals and their healthcare providers to develop a personalized care plan that outlines the services needed to improve the client’s health. These services may include connecting with healthcare and mental health providers, obtaining essential medications, assisting with housing, and accessing community resources such as food, benefits, and transportation.

Care Managers also help clients participate in community programs that provide additional support tailored to their individual needs.

HEALTH HOME CARE MANAGEMENT

A Health Home Care Manager assists in creating a person-centered care plan.  Care Managers assist in care coordination of medical, mental health, substance abuse, and social service needs. Care Managers collaborate with primary care physicians and service providers for follow-up and communication. Care Managers also ensure all parties involved are working together and sharing pertinent information to reach the desired outcome, based on goals and objectives identified in the Care Plan. Care Managers assist with housing needs, social, and community activities, and legal assistance. If hospitalized, Care Managers participate in discharge planning for a seamless transition from hospital stay to the next level of care.

Hector Diaz

Director of Clinical Services

Hector Diaz is the Director of Clinical Services at Baden Street Settlement, overseeing the organization’s Substance Use Disorder (SUD), Behavioral Health, and Mental Health programs. With over 30 years of experience in the Buffalo and Rochester areas, Hector has worked in both inpatient and outpatient SUD settings. He previously served as the Director of the SUD clinic from June 2012 to August 2021 and was promoted to Interim Director of Clinical Services in July 2024. Hector holds an Associate’s degree from Erie Community College, a Bachelor’s degree from SUNY Empire State College, and a Master’s in Mental Health Counseling from Medaille College, completed in 2016 while working full-time in the SUD Clinic. He is also a Master Credentialed Alcohol and Substance Abuse Counselor (Master CASAC). Outside of his professional work, Hector is a musician, playing both electric and upright bass with the renowned band Mambo Kings, which performs across the U.S. and Canada with symphony orchestras. Hector is also a proud U.S. Army veteran, having served during Operation Desert Storm as part of the Gulf War.

Naima Hassan
Director of Behavioral Health and Community Services

Naima Hassan joined Baden Street Settlement in July 2021 and currently serves as the Director of Behavioral Health and Community Services. She holds a degree in Mass Communication from Buffalo State College and is pursuing her Master’s in Clinical Social Work. With eight years of experience in case management and care management, Naima is deeply committed to her passion for educating, advocating, and empowering the individuals she serves.

Outside of work, Naima loves spending time with her family and friends and cherishes her annual trips to the Adirondacks each summer!
