Job Title: Care Manager Program Supervisor
Location: Rochester, NY; Monroe County
Reports to: Program Director
Department: Behavioral Health Services
Employment Type: Full Time, Monday – Friday, 8:30am – 5:00pm
The candidate must have an understanding of health homes, health and recovery plan (HARP) and home and community based services (HCBS) as well as knowledge on community resources in Monroe County. Must have basic knowledge of mental illness, substance abuse, recovery process and peer support services. The candidate must be commitment to working as part of a multi-disciplinary team to provide person centered, culturally sensitive services to a widely diverse client population.
Should have at minimum 3 years of experience of direct care in human service field or relevant experience and 1 year of a supervisor capacity. Must be a Qualified Health Professional (QHP) or other licensed level healthcare professional (e.g., RN, LMSW, LCSW, LMHC, CASAC etc.) with prior experience in a behavioral health clinic or care management supervisory capacity
Bachelors or Master´s level professional preferred; in appropriate human service field with minimum of 1 year of prior experience supervising Clinicians, Care Managers and/or other Health Care Professionals who are providing direct services to individuals with Severe Mental Illness. Substance Use Disorders or Developmental Disabilities.
Duties and responsibilities to include but not limited to the following:
Provide support and management of daily activities within the department
Work closely with team members and program director
Responsible for program development, supervision of staff and peers. Provide education and training on relevant topics to include but not limited to documentation, engagement skills, trauma informed care and best practices.
Must have a working knowledge of policy and regulatory guidance relevant to Health Home Care Management, Home & Community Based Services, Office of Mental Health (OMH), Office of Alcohol and Substance Abuse (OASAS), Department of Health (DOH) and Managed Care Organizations (MCO’s).
Must have excellent communication skills, organizational skills and ability to use multiple electronic medical records and or systems.
Must track, report, manage program assignments, transfers, referrals and assign cases
Must have a valid New York State driver’s license.
